In order to change your probation from one state to another you have to make a request through your probation officer and he will have to make a request to the jurisdiction of Puerto Rico as to whether or not they will accept you coming into their state. This will take some time for the approval as they will want to review all your records and it will be up to them and your probation officer to let you move. Why does the judge want to remove your medication? Are you an abuser of drugs? Or if you have been that could be the reason he is removing them.
Good Luck.

Most likely you can't. Transfer within the US states are allowed with permission of both states and the judge
