This is a very complicated question that would require a lot more space than is provided in this answer box. If you are thinking about starting in the U.S. you must first buy or manage a plane and work on obtaining FAA approval. If you are starting scheduled service, you will need an FAA part 121 certificate which is not obtained easily or inexpensively. Plan on spending upwards of 7 figures.

There are other certificates the FAA issues for non-scheduled, on-demand service (charter) which falls under FAA Part 135 regulations. This certificate is far easier to obtain than a 121 certificate, but still quite complicated. Plan on taking upwards of 6 to 12 months and upwards of 6 figures.

Either way, you will have to hire seasoned and experienced aviation professionals to gain approval. There are several consultants who help companies obtain their certification and I would highly recommend speaking one or several of them prior to making any commitments.

Good luck with the airline!

Start with a pile of money, a pile that reaches all the way to the sun, make sure it isn't your money, borrow it, and hire people to do all the work for you.

After thousands of lawsuits and federal manditory regulation changes you will be bankrupt, ( usually 2-3 years from staring the airline )

Not to worry though, because it wasn't your money.

The oldest joke in the airline business is that to make a million bucks, you have to start with ten million.

(ie lose nine million bucks)
