As long as we have desires in us we would continue to take re-birth
Some souls, however, do not have earthly desires left, but they still continue taking re-birth in various astral planes to fulfil their subtle desires
Finally when all desires are finished, the soul merges into the Supreme Concsiousness (God)

Nobody really knows how the souls incarnate but the Masters,representatives of the God. It is written and explained in the religious books that the soul which in this universe do moral,good deeds,love all beings may be born as humans.

Yes. I will prove it to you just now by a true philosophy of my own. I am sure nobody has shared yet with you the proof so easily and for free. Here it is: After you die, you will be in a state of unseen and indescribable element, whatever you name it like soul, aatmaa, jiva etc.. But that will be the same state as it was before your birth, and nobody in this world can deny this fact. Now that once you are incarnated from that state, similarly you will come again and again in this world, which we call it the incarnation. Are you enlightened now?

As we observe water cycle in the nature,incarnation is the similar process for upliftment of an individual in divine path. Every one is going up in the divine path(expect a few rare).As per Hindu philosophy GOD is only one. He divided himself into many for fun.By the process of incarnation the individual soul becomes part of the GOD or GOD himself.That is the freedom(MUKTI) from incarnation(AAVAGAMAN)

Yes all souls reincarnate. But the procedure is not known, not defined. And this will go on and on untill you dissolve yourself in the infinity, which is true GOD. Infinity is within you.
