If you are traveling from one country to another EVERYONE should have a passport, regardless of age. If there is any kind of war or civil unrest or any problem what so ever you need verification of who you are and where you are from.

Age specifications
Passports for applicants 16 years of age or older are issued for 10 years. Passports for applicants under 16 years of age are issued for 5 years.
All children, even babies, must have a passport. All applicants must apply in person. Parents/guardians are advised to visit the www.travel.state.gov website for detailed information regarding the passport application process for a minor child.

Yes the baby needs a passport and there are strict rules on getting passports for children. You need both parents consent. If it is an emergency, you can get the passport the same day at a passport office, but it will cost you more money. $142 for anyone under 16. http://www.travel.state.gov/passport/get/fees/fees_837.html

Yep. Any person traveling out of the US needs one. You should see the passport photo of my 6 month old daughter, it looks like a line up photo!
