As a Medical Student, in our College class, we have always been taught that the patient has the right to refuse Medical treatment, but under the case of the Criminal Justice being involved, that might be just a whole different predicament.

If you are in hand cuff's, and they are insisting you go to the hospital for a blood/alcohol test, you are probably under arrest and have no choice, if the tests come back positive for alcohol intoxication, and you are charged and found guilty, then yes you are responsible for the medical bill, and a whole lot of other "Bill's"
Don't look for others to blame. you played a game and lost. you got caught. that is the problem with people today, they wont take responsibility for their wrongs.

I am going to assume you were under arrest for a DUI. You can refuse the test and lose you drivers license for six months to a year; depending on which state you live in. Since no test was taken, There would be no bill to pay.

Probably. I suspect if you hadn't gone, and later developed serious complications, you would have sued them on the grounds that they were negligent in not forcing you to go.
