The most accurate way would be to go through your State Medical Board - they will have all of his current contact information. Most medical boards allow you to search yourself online now.

The hospital may not give out his info.

Call the hospital where the surgery was done and ask for his office phone number.

I think your best bet since you know his name would be the yellow pages online or go to peoplefinders.com
