Could be any of several problems - a discontinuity or short in the cable; a bad speaker connection inside the headphones themselves; a defect in the jack the headphones are plugged into; dirt or gunk inside the headphone jack; or simply the headphones not being plugged in all the way. To figure out which problem it is, you'll need to try plugging a different set of headphones into the same device (if the problem persists, it's in the device) and also plugging the headphones into a different device (if the problem persists, it's in the headphones). If the problem is in the headphones, look for wear at the point where the plug is attached to the cable (which is where it usually happens); also, try wiggling the cable & see if the sound stays the same. In most cases it's cheaper to replace headphones than to fix them, but if you're electro-mechanically inclined, replacement headphone plugs are available at Radio Shack.
