You should be concerned. I had very bad eating habits as a child and when I hit puberty I packed on the pounds, which then lead to other health issues (i am diabetic, anemic). The gaging, it probably just works so he does it. I used to have a client (mentally disabled) who would do that everytime she ate something green. In restaurants the waitresses thoguht she was really sick. IF it works they do it. I Would just be more forceful with other food options. Include him in preparing the meals, if he makes his own lunch maybe he will be more inclined to eat it. If he can eat candy and cookies fine I am sure he can eat other things. I am a nanny and I often let the children dip their fruit in peanut butter,. I have a lot of experience with stubborn children, if you need some tips on how to help him find other options email me! Also, don't offer him anything else excpet his lunch or dinner. Eventually he will be hungry enough to eat it. I mean don't starve him but it he misses one meal and learns you guys aren't playing around then that might help. Also try to offer him less and less pizza each day. Like tell him after he eats an apple or apple sauce that he can have a piece or something.

My daughter is 4yrs.old and I have had a very similar problem with her..maybe some of my tips will help you. My daughter will only eat chicken nuggets, macaroni n cheese, hot dogs and peanut butter and jelly sandwiches and junk food. I try to make her eat other food but she will have a tantrum and say it makes her sick or she will gag once you get it in her mouth. I've had doctors tell me as long as she is eating and maintaining a healthy weight she is fine. I've had other doctors tell me that it isn't healthy and to make sure she gets nutritional food. I used to make dinner for the family and make her what ever she will eat. I no longer do that. If she won't eat what we have for dinner then she doesn't eat.when she is hungry enough she will eat it. I never starve her but usually by the end of the night she will eat some of it anyhow. usually I will tell her just taste it to see if she likes it if she doesn't then I will make her something else but atleast she is trying things now.

The boy loves pizza, obviously. He also realizes that by gagging he can have his own way out of eating anything he doesn't want to. He is being manipulative, and his mother is being an enabler to his manipulations.
The only way to end this is to always have food on hand, but not pizza.
That way he has no choice but to eat what is given him, and child protective services can't come along and kidnap... erm... I mean protect him just because he is being refused meals (if you've got food on hand and are offering it to him, as long as it is wholesome healthy food and he has no allergies to it, they can't do a thing about it).
This may sound cruel, but in the end it is the only way to go.

It is not worth -fighting- over (arguments over something like that have to potential to drive family members apart and make the problem even worse)...but certainly needs addressing NOW. That problem will only get worse as he gets older. Sure he appears to be healthy at the moment; he's a kid afterall...but eating habits like that are very dangerous and his long term health could be seriously at risk. It is possibly a sensory issue but it also could be a case of stubboness.
You sound like a really good uncle (?)! I wish you the best of luck.

My cousin was like that - he was normal sized until a year or so ago he just started putting on weight (he's 13). If he is actually gagging - not just faking it to get what he wants, then he should go see a doctor - he might have allergies or something.
Otherwise, your aunt needs to start providing him with healthy meals and consequences for not finishing them.

let him be. he'll soon get sick of it
however, what you can do is order supreme pizzas - the ones with lots of toppings on it.
make sure he intakes vegetables and vitamins.
exercising might help because of the burning of calories.
or what you can if all fails is... go see a doctor.

You better adjust his diet before it worsens. Too much pizza is not healthy at all.
Stop buying pizza and give him healthier foods.
