The hardest thing is usually the grammar. It is hard to learn a language and if you are outside that country to keep practicing in order not to lose it. If you have a passion for it, then it all becomes easy.

It depends on your first language and the extent of its differences when compared to English. For some people it can be pronunciation due to different sounds of the language. Grammatical structure of English is certainly different then of German, Korean or any other language. So, there is no one single difficulty for all learners, everybody is different. To learn English you have to try to have as much exposure to it as possible, meaning associating with native speakers, radio and TV (for listening and speaking skills) and reading, reading, reading, reading.... anything that you enjoy. I know. English is my second language.

You need to be somewhere where you are constantly using the language, to hear, speak, read. Understanding words on paper is easy, vocabulary is easy, but learning to hear and understand, rapidly said words, to speak with the proper pronunciations, that is what is difficult. And language is not just the proper use of words, idiomatic language and slang is very important to understand people
