How do you forget about a date, an appointment that you've made with the guy your are seeing? I can see running late...but not flat out standing them up.

Just say that you're sorry, you know you screwed up, and that you'd like to make it up to him. Take HIM out to dinner and buy his dinner. It'll be a nice change.
If he tries to pull something on this date like "accidentally standing you up," then that's not cool. Then he's blatantly rejecting your attempt to make it right, and is therefore just cause for him to get dumped. (but then he'll go around all like, "well when I stood her up she broke up with me, blah blah blah.) ugh, guys.

How about being honest and asking for his forgiveness. Don't try to blame it on anyone or anything. Just say I screwed up and I am sorry. My wife drives me crazy trying to justify everything she does wrong. Just say I screwed up and I am sorry.
