he's 33, has been on efexor for 2 years, on other anti depressants for 10 years.
he is always very confused about life and himself. one day he loves me the other he is not sure of what he feels anymore. we cant make plans for the future.
he claims to have very low seff esteem, hates his job but says he doesnt have enrgy to look for an other. very low sex drive, feels guilty all the time.
what should i expect from him?

There are things you can do to help, like ensuring he eats a balanced diet and exercises regularly, even if only going with you on a daily walk,(say you don't feel safe without him there to protect you), and you can ask him to come along with you to classes that teach relaxation and meditation. If he was to undertake some activity that helped others, like writing letters in support of prisoners of conscience, for Amnesty International, or helping Meals on Wheels for 2 hrs a fortnight, or any of the hundreds of charities and good causes crying out for help, it would boost his self esteem. I was in the same condition as your BF for a long time. I also have a mood disorder. Finally, my doc found the right combo of medications for me. Now I have a more "normal" life. Hopefully, he will get there someday, too.

The choice is his, obviously, but I think a change of medication might be the answer. Has he thought about changing doctors? I went through about ten before I found one who could help me.

I dont agree with long term medication, maybe you should get him checked out it doesnt sound right. Go on line and check out the side affects of the drug. I would try and get him off it all together.
