I used to go to this church years ago. We did foot washing, but it was only during a certain time of year. I can't remember what time of the year it was. Lent? Easter? Can't remember. Anyway, it does foot washing.
Every church offers baptism. As for the Lord's table, do you mean Holy Communion? This is a Catholic church, so yes, it offers Communion, twice Monday - Saturday, and five times on Sunday.

The Seventh Day Adventist church might qualify, for while they obey about washing feet,. (John 13:17) They disobey Almighty YHVH about calling Him by heathen names (Like "Jesus" Ex. 23:13) Perhaps that is the balance you are seeking. (I don't know why you would, but if you are....)
