yes they can look at who you are phoning and whom you are receiving the calls from. But only if something is wrong then the government allows the company to look at who you are phoning otherwise I don't think they are in anyway to look at who you are calling because that's your personal details. And remember the best and trust worthy phone company is BT (British telecom). and if you think or find out that the phone company you are with are checking the calls you are making without any reason then well you can complain the reason to ofcom and the website for ofcom is http://www.ofcom.org.uk/ . And if you are with BT then for safety you should get BT HOME PRIVACY which is caller display facility and its free but you require a caller display phone in order to receive the caller display service. Yes. Every number, with the exception of toll free ones, show up on your phone bill.

Yes, Once a month they have a print out that they mail you with a bill. That is the list that they see before they mail to you i'm sure to see who your phoning. I'm sure they have easy access to it.
