Well I would pick something that is not preventable and has very little treatment options and is also devastating. So something like muscular dystrophy or Amyotrophic lateral sclerosis (Lou Gehrig's disease), autism or Alzheimer’s disease would be good candidates.
HIV/ AIDS is preventable. Cancer is preventable in a lot of cases (not all) and there are good treatments for a lot of types of cancer (though not all). Heart disease, obesity and diabetes. although they affect many people, are preventable in a lot of cases and we also have pretty good treatment options.
So after all that, I would pick AUTISM because it's the more common of the group of nonpreventable devastating neurological disease that have no good treatment options. Plus there is no cure in sight right now since we know very little about what causes it.
I like the mental illness idea that the other people wrote too -especially if you're talking about schizophrenia.

It would have to be Multiple Sclerosis. It is a killer, with no cure. My 22 year old son has it. He was diagnosed 3 years ago one when one morning (when he was away at school), he woke up totally blind. He could not see anything. His roommate called me and he was flown to the Houston Medical Center for all kinds of testing. It took them a whole day, but by the end of it, he was diagnosed with Relapsing Multiple Sclerosis. He already had 9 lesions in his brain, so it was already pretty severe. This disease is horrendous. Some days, he wakes up and has no muscle control and some days he wakes up and can't see. But then again, some mornings he wakes up and is totally fine. When ill, he has to go to the hospital for 3 days of IV therapy. He suffers so much. He takes weekly injections which cause him to be really sick for 24 hours. He runs a fever of 104 degrees and has flu like symptoms for those 24 hours. Unfortunately, this is a common side effect from this medication. If I could, I would change positions with him right this second. No one wants to see their children suffer every day of their lives. We cherish every single day. The greatest thing is the positive outlook he has on life. He lives every day to the fullest and always will.

IGNORANCE....and I truly believe that is a disease..by that I mean ignorance in the way of intolerance and just plain stupidity towards other people and cultures. I would also like to include ignorance in the way of education...there are so many who do not have the vaccine of education to cure their lack of knowledge.
If ignorance was cured then the ability to cure other diseases would definitly follow.

The common cold. I'd leave the deadly ones, since the world is overpopulated anyway.

No question: Cancer.
AIDS can be avoided, for the most part. The common cold is just that, common. Accidents happen. Mental illness is a crap shoot.
But no one, no one can avoid cancer. It chooses its victims with no regard of who you are or where you come from. There are so many varieties that encompass the word "cancer" too. Millions wouldn't have to die each year in horrible pain.
