Actually two, George Washington led the militia during the Whiskey Rebellion and James Madison directed artillery fire during the attack on Washington in the War of 1812.

At first, Teddy Roosevelt came to mind, but its not him. He did win the Congressional Medal of Honor. Then I thought "Old Hickory" (Andrew Jackson) but even though he was a general in multiple wars, they were before his Presidency. A LOT of Presidents have been former Veterans.
I would have to say James Knox Polk. The Mexican American War coincides with his Presidency. Presidents Pierce, Grant and Taylor all fought in the Mexican American War.
It is a good question and I am stumped.

None that I know of but It should be a requirement. They have the power to start a war they should have the responsibility of leading it from the front if it's so necessary. Lead by exampIe, just like the soldier's they commit, they can be replaced if something happen's to them. I doubt there would be many war's started if that were the case but you know that would never happen.
