The trouble is, it's the only photo of me with decent hair and a smile that doesn't look faked.
Should I just keep the picture on my office door?

Hehehe! You got caught with your finger in your nose while you had good hair and a nice smile. Hmmm... Sounds like you may have posed for the picture. If this is the case, I bet you have a pretty good sense of humor and aren't too embarrassed by having fun with yourself. This makes you a very likeable person!
If the office is casual and you don't see clients on an uber professional basis (lawyer, undertaker or therapist) keep the picture up only for a few days while everyone is laughing.... Enjoy the popularity! Once the picture has been forgotten about, take it down and resume being your fun loving self!

I think you should get rid of it, hope no one saw, and if they did, find a new job. Or you could laugh along with it and act like you don't mind (keep it up as a joke on yourself) and you won't look bad, but funny.

Leave it and show people you sense of humor. everyone loves to laugh. if you feel comfortable leave it, that is really all that matters.
