I read with concern about what you are going through. You need to get a proper and accurate diagnosis. If your dermatologist is not helping, then look for another.
It can be hives or any bacterial or fungal infection. You need the correct medication to deal with it. It may not even be antihistamine. You may also need a stronger prescription.
You should not have to bear your pain for 3 years. It is far too long.
You may also need to work out what is triggering it. Is it stress? Is it food related? Is it dust mites? Go through an elimination process.
Once you get the proper medication for it, then you ought to be on the road to recovery. This is when you divert your time and energy to looking for natural remedies to help reduce the chance of your itching coming back.
I am an eczema sufferer and know what it feels like to be itching, with no control and unable to get sleep.
Your case may not be eczema but here is a newsletter support site that you can also get some free tips on relieving itching http://www.eczematreatmentsecrets.com

Go to your family doctor, make sure he knows this itching has been going on for some time. You need to have a complete blood workup, with hepatic panel done on your liver, for this sounds like a liver problem. Does prednisone stop the itching?
You could also have something wrong with your thyroid, this will show up in the CBC blood work.
Usually something like this is not from the outside, IE soap, laundry soap, bath soap. But is a symptom of an underlying condition. If your family doctor cannot find the cause, ask for a referral to a specialist. Caution, do not get pregnant until you find the cause. If something is wrong, it could prevent you from having a healthy baby. Find the cause and get healthy first. Please do not let this go, if ignored the condition could become serious.

I have recently been experiencing this same thing! I have been itching like you describe for about two months now. I finally caved in and went to the doctor who gave me an alergy shot and higher potent antihistamines. About the only thing I can do now is go through alergy testing (I moved to a new area so something is bothering me)...then weekly shots.
I had no real rash either, just the feel of a rash, the whole nine yards. My Doc told me that it is systemic and we have to find the nasty culprit and then take shots to reduce the reaction to it. Good luck - I feel your pain!

Itching can be caused by insect bites like mosquito bites, heat rashes or allergic reactions.Aloe vera gel , apple cider vinegar or vitamin E oil can be effectively used on the affected skin.

Go back and see your doctor. There's other reasons for itching, besides allergies, for example, problems with the liver. It's not fair that you have suffered this for so long, and you shouldn't have to put up with it for any longer. There's a reason and remedy out there, just keep at it. Good luck......HONEYPOT has a good point

Sounds like hives.
You can get if you are allergic to washing power, soap, exercise.
it doesn't last long but is sure can itch.
I get it when exercising, I have had it for 40 years now and I still take antihistamines..and it doesn't stop you from getting pregnant.

You need to go to an allergy specialist. What you are experiencing is an allergic reaction since the antihistamine makes it go away.
