GO to IRS.gov and print out a 1040-X. There should be instructions with it.

You use a form 1040X - download it and the instructions at irs.gov

The form asks for information on what you originally filed, the corrected info, and the difference, and there's a section for you to explain what you're changing.

just take your info into your tax pro and ask them to do an amendment for that year. pretty simple
