Chocolate is made up of cocoa powder (solids) and cocoa butter. You do not need the cocoa butter for the chocolate milk, but you could use it as well.
If you would like to make regular hot cocoa then take 1 cup of milk heat it to almost boiling (I don't like the taste of scalded milk) Add 1.5 tbsp of cocoa powder and 1.5 tbsp of sugar. This is your traditional recipe for 1 cup of hot cocoa.
You can make your own mix by combining the ingredients in advance. Then add 3 tbsp of mix per cup of milk.
You can vary the taste of the drink by adding syrups, liquors, or extracts.

Usually you will have to heat the milk. Put some milk in a pot and bring it until its steaming, then add in some cocoa powder, to the color and intensity of choclate you want. Then add sugar to taste into the hot mixture. After you have let it incorporate(maybe like 1 minute of mixing) then take off the heat and let cool and chill. Good luck!

You could add melted chocolate and sugar, or chocolate syrup or powdered "chocolate mix" which both already have sugar, but personally I'd add a bit of cocoa powder and sugar to my milk (heated, for hot chocolate... preferably with a few melting tiny marshmallows on top). Most "chocolate milk" is made with whole milk or 2% milk, but you could use lower fat too.
