You would be big enough for the adult dose. Usual dose is 250mg to 500mg. 3 to 4 times a day.

Whatever is prescribed by your doctor. If you are sick and got somebody else's leftover prescription then don't bother.
Antibiotics are prescribed based on the causitive organism of an illness, as some are resistant to certain antibiotics and susceptible to others. Also, if you have a viral or fungal infection then antibiotics will do nothing for you.
Also, when you are on antibiotics make sure to take the entire prescription, or you could select for the strains that are more resistant.

You should take only what is prescribed by your physician. If you're concerned about a dose, ask your pharmacist to double check it.

The dose will vary depending on the condition and bacteria being treated.
