File Form 8839 with your tax return to claim the adoption expenses credit. You can get copies of the form and instructions at the IRS website.

you would first need to fill out form 8839. the adoption tax credit calculated, if any, would go on Form 1040, line 54.

you probably did line 15 on style 8889 incorrectly, i think of. 'unreimbursed' in that context ability no longer paid via any outdoors source -- corporation, well-being plan, etc. -- and would contain those qualified expenses which you paid.
