Your former employer may be able to give you one or at least the #s so that you can file a replacement W2. I had the same problem. If no info is available, have your accountant make an estimate from your past pay stubs..the odds are that you will be fairly close.

Your employer. If they have a payroll department, that would be the place to start. If they don't, ask your boss.

Request a copy from your employer. The are allowed to make a reasonable charge for a duplicate (few do that but they can) but they must supply it upon request.
