I would say, just above the salutation, between the salutation and the date. The reader would get his attention drawn to this as he would read the subject of the letter first. The type to be used should also be slightly bolder or in capitals than the type used for the letter.
Of course this is not a rule, but this is what I do.
Also, you can write 'Confidential' on the envelope too, so that the addressee would know that it needs to be opened by someone responsible, or by the designated person.
