Most of them do not but the people who try to qualifiy for the last few spots. They have to pay to play in those.

If they have a PGA tour card, or if they are exempt, they do not have to pay to enter.

If they 'Monday qualify', which is competing for the last few tournament spots, they need to pay a tournament entry fee.

Non-members do but tour players who went to Q-School(a tournament where the top 30 finishers after 108 holes qualifies to play on the tour) or players that get exemptions do not.

Players on the professional circuit tours do have to pay entry fees to become tour players(they are professionals as they playing for prize money and are allowed to get endorsements which would be a clear violation of the NCAA amateurism rules). These tours are called the Pepsi Tour or the Tides Tour. There are also long drive tours and putting tours.

Negative, all they need is a valid PGA card, some even get appearance fees.

no but they do have to pay other people, like their caddy
