Most land line phone companies charge a fee to use 411. The good news is that many people have a monthly package that allows you to call 411 a certain number of times (usually like 3 times before you are charged). Call the billing/service number for your phone company to find out for sure.

I don't think the why, how, or time matter.
Some companies charge for operator assistance.
Just depends on your area and service package.
Check your bill for reference, or call them.

i always use superpages.com for information.. why pay someone else to look it up on a pc when you can do it yourself?
