What would be a good nurse diagnosis for a patient that is dizzy, lightheaded, SOB, HR 138 Respirations 28 no other sx. noted

This is another students assessment not mine :) can you tell we just started nursing school!!! LOL

I agree with the previous answer.
Definately hypotensive.
Or even an anxiety attack is another possibility.

The description is too brief to determine an adequate nursing dx. Is the patient suffering from anxiety? Asthma? Have ABG's been assessed? BP? M/F? Temp? EKG? Curent meds? There are too many other variables that were not answered to provide a correct nursing diagnosis. It does, however, appear there may be some "ineffective gas exchange" happening. From there, who knows ???

There isn't enough information to give a good diagnosis. Whats his blood pressure? How old is he? Approx age and is he overweight? Sweating? How is his color? Does it come and go as he is up working or the majority of the time? What type of work?

What is the temperature outside? what does he/she do for work? How old is the patient? Why do you want a nurse diagnosis and not a doctors? You left too much info. out!!

Well...theyd be at risk for falls because of the dizziness, if theyre SOB with 28 RPM they have abnormal respiratory status....its just common sense really.

My first thoughts were the following. Dehydration, hypotension, or low blood sugar. Without any other s/s its all I could come up with. Good luck too you.
