How come the dentists don't recommend warm water for teeth brushing? It's recommended for washing dishes, counters, clothes, etc. to kill bacteria and germs. Does anyone think it would be more effective to use warm water with toothpaste or that it doesn't matter.

Warm water doesn't kill bacteria, so even if you wash dishes or clothing it doesn't matter- it's the same as using cold.

In order to prevent gum disease you have to get the bacteria out of you mouth entirely by brushing and flossing really well. Some rinses work well too, but nothing is better than flossing between your teeth & brushing your teeth and tongue.
