What kind of insult is "bacalao"?

Does it have a real meaning (like, say, "cow" in English"), or is it just an insult, on par to something like "snot nose"?

If someone called you "bacalao", he/she meant that you "stink"...sorry!

It does have a real meaning:
It means cod (fish).
