i wouldnt feel comfy with my ex being friendly with her ex and exchanging gifts...you oviously trust her a lot and have full trust in your relationship, which is great....
i would suggest something not very personal, like a book, or a pen or a c.d. maybe a key ring, depends what her buget is and the age of the ex.

It really depends on how close the relationship is between them. I'm still friends with some ofmy exs so i would get them a friend gift. And then there are those exs that once you break up its just bumping into them at various places and making small talk, or the exs that you wish that they would move far far away-so you would never see them again.These id probably wouldn't care if it was there bday at all.

It depends on why they are ex's and why she is giving a gift.
There are too many variables.
If it's just to say, "Hey, glad we can still be friends.", I'd say something like a gift certificate to Starbucks, or a CD, or a gag gift, but nothing really personal.
