The nurse is incorrect. Jock Itch is ringworm of the groin. It causes irritation, and when mixed with sweat, will eventually smell.
It does not, however, cause bad breath.

Make her up a gift basket for Christmas. Fill it with bubble bath, a loofah, body wash, lotion, deodorant, tooth brush, toothpaste, perfume, razor, shaving gel, and maybe some makeup. Make sure you get the stuff all in the same scent. Kind of like a 'spa' package. (This can be done very cheaply if you have a dollar store in your area, if money is an issue) I'd suggest also putting a little jewelry item or something in it so it doesn't seem so overtly obvious that you think she stinks. Hopefully she'll use the stuff. I'd wait a couple of weeks after giving it. If the problem persists then I would gently talk to her and tell her you don't want to hurt her feelings but she needs to take care of her hygiene. If you're really uncomfortable talking to her then ask the school nurse to talk to her. That way the cousin won't know who it was and maybe she'll realize many others probably also notice her issues. It just may embarrass her enough to take care of herself. I had a very good friend in school who had hygiene issues. I overhead some others talking about it and chose to talk to her. She confided her family did not have the money for hygiene products. My mom and agreed to buy extra stuff for me to give her. I spoke with the school nurse and got permission for her to use the shower at school (her water at home was often turned off) without others knowing. She used to sneak off during study hall and take care of her needs. She was very grateful and I felt really good to be able to help out a friend.

It can cause an odor in the crotch area only. It's a rash that can cause minor damage to the skin. Your body's immune system fights the area of the rash and causes a secretion that may emit an odor. As for the other areas, it's not related.
