Mostly your age, unless you list several illnesses you had or there is something in the family history that would raise a caution flag.

It's usually based on age, if the person smokes, (in the case of co health insurance provider)
If this is a personal plan, they could also consider past claims (history) etc..

It depends on the state you are in and their regulations. In CT, if you're talking about individual coverage, underwriting applies (you can be declined). Rates are set by area, age, and level of coverage (# of dependents covered).

In group coverage, there are generally no pre-existing conditions, so it is guaranteed coverage. They take into account your area, age, and level of coverage. Rates are based on the claims by 5-year age bands and the insurance companies' estimate of what it is going to take to insure you.

Generally - your location, the type of coverage you're looking for and how much out of pocket to the provider you're going to pay, whether or not you're married and/or have children to cover.

Age and pre-existing conditions SOMETIMES come into play, but only if you're purchasing it yourself and not part of a group.
