Depends on department regulations and the style of chain. Some are lanyards and are worn around the neck. Others have a ring on one end of the chain. These are typically worn with the left breast pocket buttoned through the ring.

I wear mine on the right side of me, just hooked underneat that little flap on my shoulder, and tucked into my pocket closest to my arm pit. That is how my department wears them.
