It would only be a mutation, because you would be the only person with a third eyeball. If, for some reason, that third eyeball proved useful for survival and eventually, over thousands of years, spread throughout the human population, that would be evolution.

No. It is proof that your "Intelligent designer" likes to sip the sauce a bit.

Seriously, though, deformities (not that you seriously have one) are part and parcel for evolution theory.

If one chooses to believe in intelligent design (ID), then one would also have to include the possibility of a retarded designer. For every wonderful design that IDiots cry out as evidence of intelligence, there are several factors that show that the design isn't so wonderful. The eye is a great thing, but IDiots can't offer a valid, acceptable explanation for glaucoma, cataracts, near/far sightedness, etc. That is just one example of not-so-intelligent design. You can visit http://www.edwardtbabinski.us/babinski/designer.html. Babinski is a former YEC that finally chose common sense and intellectual honesty over biblical literalism. He almost lost his faith because his YEC cohorts didn't allow him a choice to believe in both the bible AND science.

Of course, YEC's will cite "The Fall" as the reason for health ills ever since. However, it doesn't explain why animal fossils show clear signs of disease and injury, millions of years before man ever walked the earth. Neither does it explain why animals would suffer a "fall," either, since animals do not have souls and CANNOT sin.

no, that would be called a mutation. But if you notice most animal life on this planet has a face, two eyes, two nostrals and a mouth.

Oh, and while we're on the subject of Adam and Eve... naahh, you've probably heard the story.
