I live in the Appalachian Mountains and this is a phrase I have heard all of my life. "Smack dab" is sometimes put with other words like "smack dab on it." It simply means "right." It is like saying "Right in the middle." For example, "I was smack dab in the middle of doing my homework, when the phone rang." It has been used for many years and it definitely is a southern expression.
