It means you're welcome to their assistance. It's lost its true meaning over time i'm sure, now its just accepted as a reply to thank you.

I guess that is stardard. I don't always. I feel weird saying it for some reason. I like to respond with a thank you back! I get made fun at work, my higher ups and give me work and they say thank you and I am like thank you, and they go no really thank you! We all just laugh. That is me though. I am crazy.
