who is unwell, and to give her a
pot of butter (real!) and a cake (very rich!). In my opinion, my
grandmother must cut off on cholesterol!
But I do not know how to tell my mother as not to offend her (she is
very proud of her cakes).

Depends on your age. If you are young and still living at home, try discussing your concerns with your mother, but deliver the cake and butter as told. If you are grown, be sweet and kind to your mother as you gently refuse to run this errand and tell her that your are refusing out of love for both her and your grandmother.

how about just a small slice of cake? no butter. you're obviously old enough to take yourself to your grandma so you're old enough to tell your mom your feelings. tell her what you think and why. offer a compromise. if she does get upset about it, lie to her and say grandma loved the cake.(its just a little white lie)

by all means do . its up to grandma what she eats and if she likes cholesterol she shall have cholesterol . but , its my idea what she wants most is just to see your face and know that you are doing well. go and see her for you first , her second and your mother third . 3 birds with one stone , thats hard to compare to. trust me .
