Most tattoo shops have consent forms that they use, why not pick one up from there? Oh wait...they probably won't tattoo you anyway because you are underage and sorry buddy, but your parents usually has to be present at the time the tattoo is done, signing a form and bringing it in won't cut it. This isn't a middle school field trip its a tattoo. Just wait another year, it won't kill you I promise.

You dont need a consent form nor will any upstanding tattoo shop accept it. The parent must be present with valid photo ID when you get the tattoo.

its not even a matter of signing the form, or even them signing and leaving. your parent or guardian legally HAS TO BE ON THE PREMISES while you are being tattooed. And BOTH of your ID's have to be copied onto it at the same time. Just getting them a copy of a form won't cut it. How do we know you didnt either photoshop the ID or sign it yourself! Sorry tough luck

I think that your mom has to go with you and sign it in front of them...can't bring the form in pre-signed already. I would think that the studios would have the forms available on file if they will do underage tattooing.
