Your awards and decorations will be listed on your DD Form 214. This is the separations form they gave you when you processed out.

If you have already been discharged, they will be listed on your DD Form 214 (Certificate of Release or Discharge). If still on Active Duty, they will be in your personnel file which is maintained by your S-1 or Administration/Personnel section. They can let you know.

Submit a request to the National Personnel Records Center, St. Louis. You will need the SF-180. http://www.archives.gov/st-louis/military-personnel/standard-form-180.html .

Look at your DD214 (discharge papers). All awards are listed there. Copy 4 also lists the nature of your discharge.
