First of all be prepared to take no for an answer. Women mean no when they say no. So you need to learn if this woman has any interest in getting back together. If the answer is no, move on to more fertile soil.
Secondly, tell her how you feel in a plain respectful and honest manner. Women really appreciate honesty because they feel they get so little of it from men. Be a man and profess your feelings. Take a chance but be ready for rejection if it comes. If she does reject you, don't get angry - be grateful that she heard you out.
Third, if the interest exists, address the reasons that led to your break-up. If you were always late, try to be on time. If you were overbearing, try to lighten up. If you were jealous, give her room to breathe.
If you want a good girlfriend, you have to be a good boyfriend.

you can't get a person to like you again, but you can let her know that you still have feelings for her and ask her would she consider being with you again and ask her how she feels about you.
