I would think that a rural doctor would have to be more of a generalist and see a variety of problems, whereas the urban doctor can specialize in a specific area of medicine.

Statistically, rural doctors are in private practice and most doctors in the urban area belong to a corporation and work for PPOs and HMOs providers.

Mostly the size of the buildings. Rural areas usually have more dirt roads too.
