The simplest would be :

Mihi ignoscas

Just having the verb in the present subjunctive as the main (in this case only) verb would handle it all.

Zlevad's are correct, but very formal.

The 'Redona" in one answer is the wrong verb to use for 'forget' in this sense.

For goodness sake, Latin is the language that was once spoken in the Roman Empire! Not Latin American Spanish which is exactly that: Spanish... I can't believe it!

You can either translate by the equivalent of
" I implore you to forgive me" ~ Obsecro ut mihi ignoscas

or " May it please you to forgive me" ~ Mihi ignoscere tibi libeat
