The federal form is a IRS Form 2441 if you want to claim the child care benefit. The state form depends on the state you live in.
Unless your state has some unusual requirement I don't know why the day care provider would need the SS# of your child unless it is to provide you a receipt for the amount that you paid for that child.

I believe that a SS# may be required if they daycare provider gets any payment for your child from a state/government agency like Family Indepedance Agency. But that is something they should already have when the child began going to the daycare.

Nothing requires a child's SS number. Don't give it out - almost sounds like a scam or they want to use it for some illegal to do their taxes with. Too many kids are getting their first job and find out that they have been working for years.

both the mother or the daddy - or worse both - stay in worry of each and every thing (probably because of the information, or PTA memories of negative human beings, crime, drugs, etc.) So the little ones are coated by technique of secure practices 24/7. i imagine once a million undesirable apple no count if that's in another usa or no longer scares some dad and mom into action then this tale spreads to electrify different dad and mom, and harm the lives of danger free human beings. The in the direction of homestead the more desirable scared they develop into. the options with todays quickly-paced friends is ignore them, get to draw close them in the previous they pass again as your shifting too. in case you do pass to suburbia - the position you recognize all and sundry will be for awhile you particularly can meet your friends and performance friendships that very last. won't be able to peg down u . s . a . because it really is so huge, that's referenced from San Francisco bay area perspective. There are huge numbers of human beings on the pass in California, ties type without delay, yet human beings pass many times. Relationships are frustrating to commence, in case you recognize you both will be shifting quickly. As for the treatment of gays and minorities - in fields that contain little ones - you may not in any respect wager how human beings will react, yet I many times plan for the worst, if the Alpha parent of the school, or PTA, is chuffed then the flock will be too. unhappy psychological image yet I view human beings more desirable as followers than persons or leaders in those situations. you ought to draw close the guy in the previous condemning them.

No daycare provider has any need for a child's SSN! You need the providers Tax ID # (SSN, TIN, or EIN) to claim the Dependent Care Credit, but the provider has NO NEED what so ever for the child's SSN.

Well, if you are receiving assistance from the state to pay for daycare, they may need it for that. I've never given my daughter's out though, and I did get child care assistance while I was in school.
Otherwise, you could tell them that you need to see the form they are completing and tell them you will have your "accountant" complete it.
You could also call the IRS directly.

Your question either is or is not. Does that make sense? Your question does not either.
If you pay for daycare then who ever does your taxes should know. Don't do the taxes yourself. If it cheap to hire a good tax person. If you run a daycare center then again do not do the taxws yourself. Hire a good accountant to watch over your business.
