The index is the pages at the end of the book that tell you where to find information about something specific IN the book. Like if it's a biography of an author, and you want to know where her first book is mentioned, you find its title on the list and it tells you pages 13, 157-167 and 204.

The bibliography is the list of books and other sources the author used in researching the book.
