Depends where you are going from and to. I would recommend the following:

Virgin, Quantas, Singapore, Thai Airways and BA.

I find BA fantastic on shorthaul, there is loads of leg room, they have improved their inflight food and I have never had any issues with delays or lost luggage.
Virgin are really good to the States, they have brilliant inflight entertainment, you can choose from over 50 movies and watch them whenever you want. The staff really look after you and keep you fed and watered all through the flight.
Thai are fantastic. Although the plane we flew on was older, because there were no tv screens in the backs of the seats there was so much more room. Some people may not like this, but I found that if someone in front reclined then it didn't make any difference, I would rather read a book and not be disturbed than have a couple of films and someone reclining in my face. The staff were dressed traditionally when we arrived and were so courteous. The food was wonderful and we had a great flight.
Quantas are very good. The flight to Australia was very smooth. We were given goody bags with food in to keep us going in between meals. A great idea! I would only use them now to go to Australia.
The worst airline - has to be Monarch. We have flown with them to the Maldives and Kenya and both times have been disasters. Their planes are cramped and really only should be used for short haul. The staff are non commital and don't really care, we paid for upgrades on the flight out and because the plane they were planning to use developed a fault we didn't get the upgrades. There was no apology and no refund and half the people in our situation were placed in the wrong seats and got the wrong meals.

The best way to judge this in my opinion is not on the luxury or price but the airline safety record, which you can checkout by looking on line with Flight International

Singapore airlines at the top of the list with Air China at the bottom.

Depends where you are going. I go to the Far East a lot and all the airlines I've travelled with have been excellent (Cathay Pacific, KLM, Lufthansa, Asiana)
I think short haul flights may have more variation in quality.

In my experience, Virgin Atlantic are very good. The inflight entertainment systems are good, and the service is also impressive. I like the aircraft they operate, too. I think a 747 is tops, lots of space to walk around.

A few years ago, I flew with Iceland air, between London and New York (changing at Keflavik, Iceland). What made that good was every stewardess was totally fit. Every one.

Depends where you are and where you want to fly to... There are cheap and cheerful airlines - pretty much no frills but sometimes bargain prices or there is full on luxury with the bigger airlines. Best I've flown (I live in the UK) is Emirates - brilliant.

I recommend Singapore airlines & United Arab emirates airlines. Their services are greats, friendly & accomodating staff.

Certainly aim for one with at least one person of European descent on the flight deck. At a pinch, an American will do.
