You really should since it is the title of a book but most writers do not since it is certainly recognized as a book title whether underlined or not. Same goes for the Koran (also spelled Quran or Qur'an). I doubt that most English teachers in college would mark it as a mistake if either of these were not underlined. I am an English teacher in university and I would accept them underlined or not.

By the way, most Internet writers do not use underline for book titles. They resort to quote marks just the same as titles of poems and magazine articles or Italics which, by age old standards does not equate it with titles of any other category. What's the world coming to? Or perhaps I should say "To what's the world coming?"

Good question. I would say, Yes because it is a book. It is also a collection of books. The Bible literally means "The Book".
