anorexia nervosa is an eating disorder that occurs usually in teen age girls,lots of work have been done on this topic specially in the westren world because it tends to happen there.
you should first read about it in one of the textbooks of psychiatry to get a better understanding of its statistics,i would like to refer you to one of the best books on this topic(the comprehensive textbook of psychiatry by caplan and saddock).

Some people date it back to some famous nun or something, she starved herself for god. But that wasn't really the mental state anorexia. That was sort of different. I believe it's been around since people were able to have low self esteem. But probably not recogized, till the 1800's, even then, it wasn't really recognized as a deadly disorder. For real documents, it probably won't show up until the 60's.

As long as people have been around to criticize each other, the threat of low self esteem has been around.
Anorexia is caused by low self esteem.
