For concrete drilled shafts, the governing codes required that the entire shaft be reinforced. As a practical matter, the rebar cage is held off the bottom of the drilled hole about 6-12" and is held back from the sides of the hole a minimum of 3" clear or 6" clear if the slurry method of shaft construction is used.

I don't think so but the code never says that. would extend at least one bar to the bottom but you could end the rest of the steel when the bending moments become negligible

Ditto, it seems there would be no bending stresses at the bottom of the pile.
