If you're talking about a US passport, you can download the application and all the info at: http://travel.state.gov/passport/get/first/first_830.html
It takes about six weeks for a US citizen to get a passport... so says the Passport Services Office. To obtain a passport for the first time, you need to go in person to a "passport acceptance facility". You can get one faster if you live near one of the 13 regional passport agencies. You must make an appointment for this service. For more information to http://travel.state.gov/passport/passport_1738.html It always pays to get your passport early to avoid that stress if you can.
