This is a complicated problem in terms of the money situation. Has she been declared incompetent by the doctor and legal system. If so the social services may be able to help you out with a lot of the paper trail that you have to faithfully complete and always keep a copy for yourself. If her disease is progressive than you have time to get the process going with the help of your family doctor, hopefully sympathetic and competent to fill the forms for the social worker at the hospital where you seek medical help.
Your grandmother should have some sort of medicare if she had worked, from her taxes she paid into over the years. They will of course ask about her earnings/savings/property/$$$ before the system can offer the help you need. If she has no savings or earning then the social system can offer medical help through the state, it also depends on how long she has been a resident in that state.
Seek your family doctor help and then go for your self to see the social services, bring her papers and income taxes forms/her bills e.g. phone, power, rent, taxes documentation. Make an appointment to see the social worker.

I wonder if you really mean Medicare? My understanding of Medicaid is that there is only one plan, no choices. California calls their Medicaid Plan "Medical", other States may have unique names also. It is for all persons who have little money. I would call the New York Medicare office and hopefully you can describe her financial situation to them. Some States also have Geriatric Assessment Teams, they are an excellent resource and are financed by the Govt.

There is a nurse at the senior center in our town that helps you choose these things. There is also a Resource guide in the front of the phone book.It has many different places you can get help. Also try the Alzheimers Association. 1-800-337-3827.
