The major problem common to all these systems is very old design that is not at all good for modern urban users. The basic design has not changed very much since Roman times and thats not good either. A modern water system should be based on good engineering concepts and thats not the case anywhere on the planet. Anyway, as a start seawater should be frozen in huge quantities and the ice melted for use in urban systems. This may seem a stupid idea but its not.

I am guessing that you are asking what technical and management problems are in municipal water distribution system.
The answer to that is: fecal contamination, rust build up, calcium circulation, and animals, just to name a few. Thousands of them really.
